Frontline Mobile Phlebotomy (FMP) is a privately owned business that conducts services through various forms of specimen collection, the processing of specimens and specimen delivery to the laboratory requested by the ordering provider.
FMP is not a specimen or diagnostic testing laboratory.
FMP cannot perform any form of specimen collection without a valid laboratory requisition given to the client by the ordering physician.
FMP cannot place any test orders on behalf of the ordering physician nor add any additional tests.
FMP is not responsible for any charges sent to you by the testing laboratory.
FMP is not responsible for verifying the clients insurance or expiration, as that is the responsibility of the insured individual and their doctor.
FMP will only charge for the services provided and will collect all fees immediately after services.
1. The possible risks associated with blood drawing are pain, bleeding, fainting, bruising, infection and/or hematoma (blood clot under the skin) at the injection site.
2. Having blood drawn may produce discomfort or minor bleeding and the possibility of bruising at the site of the needle puncture.
3. There is also a slight risk (less than 1 in 1,000) of infection at the site of the needle puncture.
4. Although rare, some people have experienced nausea, lightheadedness, and fainting in association with a blood draw either during or shortly after the needle is inserted.
5. If you develop bruising, this will go away after a couple of days and can be treated with warm compresses and/or medication.
6. You may experience side effects of having your blood drawn. These side effects can include pain, swelling, bruising, tissue discoloration or scarring around the vein that is used to draw the blood sample.
7. There is a possibility that some of these tissue changes and scarring could become permanent.
