Payment Policy

  • FMP requires a deposit on the day of scheduling your appointment. *If deposit is not paid on the day of scheduling, the appointment slot will be forfeited and made available*
  • FMP guarantees after the deposit has been paid your appointment slot is secured. 
  • FMP will send an appointment confirmation after your deposit has been paid.
  • FMP will issue an invoice with proof of payment after every payment is received.
  • FMP does not accept checks. *Under special circumstances, it may be allowed*
  • FMP does not refund deposits if cancellation was made after the 24 hour cancellation policy.
  • FMP will only charge for services provided to you upon agreement of scheduling your appointment. Any additional invoices received by the following (E.g. Laboratory, insurance fees/balances) will be the responsibility of the individual receiving them.
